this doctor is just the number one doctor, he is the number one number one doctor, better than any old just number one doctor, but perhaps its a couple of doctors, being plural and all, they are both the number one doctors, but through laws of logic, there cannot be more than one number one, so technically one would be one and the other would be two, but the better one obviously does not want to be mean to number two
